We are seeking to hire an experienced Registered Nurse to join our ever-growing interdisciplinary team of Home Health and Hospice professionals.
Our mission is to empower our clinicians to contribute to the health and well-being of every patient placed in our care, throughout their lifetime, by utilizing evidenced based best practices.
RN Case Management Expectations :
- Provides direct patient care in their home.
- Oversees the patients’ plan of care.
- Coordinates the patients’ ongoing skilled care under the direction of patients' physician.
- Promotes positive patient outcomes.
- Participates in patient interdisciplinary case conference.

- Annual salary ranges from $75,000-$85,000 depending on years of experience
Requirements to join the team as a RN :
- Registered Nurse license required
- Ideal candidates will have home care experience but we are open to training RNs with a ICU / Med-Surg / Emergency Room background
- Valid driver's license and auto insurance
