HEALTHY START PROGRAM MANAGER

Duties and Responsibilities :

  • Ensures that perinatal patients are adequately assessed using required tools and given appropriate support and interventions to reduce low weight births, preterm delivery, and late entry to perinatal care
  • Ensures that perinatal patients are given evidence-based interventions and education to positively influence behaviors that impact birth outcomes, such as, smoking cessation, substance use cessation, early entry care, consistent attendance of appointments and specialty referrals.
  • Identifies gaps in services to address social determinants of health based on outcomes and patient feedback and assists in developing plans, community resources and other partnerships to meet identified needs.

Shares resources with other COH staff on external social service agencies to meet needs of patients for housing, legal, domestic violence, substance abuse, etc

  • Ensures that department staff members are collaborating effectively within the department by reviewing outcomes on a monthly and quarterly basis , as well as patient feedback, and works to resolve barriers to effective patient care
  • Establishes data collection standards and reviews data collection efforts by staff monthly to ensure accuracy and maintenance of effort.

Contributes data monthly to the Child and Maternal health data dashboard

  • Monitors grant deliverables for the perinatal care coordination components of Healthy Start grant to ensure that commitments and requirements are met
  • Provides updates on client successes or program outcomes for fundraising and communications purposes as needed
  • Conducts weekly supervision with each Healthy Start perinatal care coordinator and doula, and provides ongoing feedback and support, including opportunities for professional development
  • Reviews the performance of team members in a timely manner, including completing 90 day and annual performance evaluations per their due dates and documenting any issues or changes
  • Complies with and helps to enforce standard organizational policies and procedures as described in the COH toolbox including but not limited to the employee handbook, the Accounting Manual, and other COH policies and procedures
  • Promotes the One Community, One Hope culture through participating in quarterly culture initiatives and implementing teambuilding activities or creating other opportunities that promote a positive workplace culture.

Minimum Qualifications :

  • B.A. in Human Services related field (social work, early childhood education, family studies, psychology) or equivalent professional experience required
  • At least 1 year of supervisory experience required; at least 2 years of public health and / or human services filed experience required
  • Experience with providing public health case management and education (i.e., reproductive health, smoking cessation, child development, and parenting) to at risk populations strongly preferred
  • Demonstrated cultural competence in communicating with underserved, diverse populations
  • Knowledge of community resources or the ability to become knowledgeable required
  • Strong interpersonal, organizational, analytical, problem solving and cross-cultural sensitivity skills required.
  • Ability to work flexible hours, including weekends and evenings, required.
  • Ability to conduct home visits. Valid driver’s license and vehicle required, as well as proof of auto insurance required.
