We are seeking a dedicated RN Clinical Supervisor to join our Home Health team. You will oversee and guide a team of professionals in providing quality care to individuals in the home setting.
This role requires clinical expertise, experience with chart audits, and exceptional time management.
Responsibilities :
- Monitor and evaluate the effectiveness of care plans
- Communicate with collaborating physicians or specialists regarding patient care and changes in condition
- Educate patients about health maintenance and disease prevention
- Facilitate referrals to other healthcare professionals and medical facilities
- Ensure the accuracy of patient medical records
- Provide advice and emotional support to patients and their family members
- Other duties as assigned
Qualifications :
- Familiarity with medical software and equipment
- Ability to build rapport with patients
- Strong problem solving and critical thinking skills
- Ability to thrive in a fast-paced environment
- An active, unencumbered RN license required
