Job Overview :
We are seeking a compassionate and skilled Home Health Speech Therapist to join our team. In this role, you will provide personalized speech, language, and swallowing therapy services to patients in their homes.
You will work closely with patients, their families, and other healthcare professionals to develop and implement effective treatment plans aimed at improving communication and swallowing functions.
This position requires strong clinical skills, excellent communication, and the ability to work independently.
Key Responsibilities :
- Patient Assessment : Conduct thorough evaluations of patients' speech, language, voice, fluency, and swallowing abilities to diagnose disorders and determine appropriate treatment plans.
- Treatment Planning : Develop individualized therapy plans tailored to each patient’s specific needs and goals, incorporating evidence-based practices.
- Therapy Implementation : Provide direct therapy services in patients' homes, focusing on improving communication, cognitive-linguistic, and swallowing functions.
Use a variety of therapeutic techniques and interventions.
- Progress Monitoring : Regularly assess and document patient progress, adjusting treatment plans as necessary to achieve desired outcomes.
- Collaboration : Work collaboratively with a multidisciplinary team, including physicians, nurses, occupational therapists, and physical therapists, to coordinate care and ensure comprehensive treatment.
- Patient and Family Education : Educate patients and their families on communication strategies, exercises, and techniques to support ongoing progress outside of therapy sessions.
- Documentation : Maintain accurate and timely records of patient evaluations, treatment plans, progress notes, and discharge summaries in compliance with healthcare regulations.
- Compliance : Adhere to all applicable federal, state, and local laws and regulations governing home health care, as well as company policies and procedures.
Qualifications :
- Master’s degree in Speech-Language Pathology from an accredited program.
- Current state licensure as a Speech-Language Pathologist.
- Certificate of Clinical Competence (CCC) from the American Speech-Language-Hearing Association (ASHA) preferred.
- Minimum of 1-2 years of experience in speech-language pathology, with home health experience preferred.
- Strong knowledge of speech, language, and swallowing disorders across the lifespan.
- Excellent communication, organizational, and interpersonal skills.
- Ability to work independently and manage a caseload in a home health setting.
- Reliable transportation and a valid driver’s license.
Physical Requirements :
- Ability to travel to patient homes within the service area.
- Ability to lift and move therapy materials and equipment.
- Capacity to perform the physical demands of the job, including standing, walking, and sitting for extended periods.
