Behavioral Health Consultant

The Behavioral Health Consultant is part of the person-centered integrated care team and provides direct and indirect services to persons served in the medical services department.

This is a full-time, benefit eligible position working 40 hours weekly.

Brief Assessment and Behavioral Interventions

• Provide routine consultation (in person and remote) to primary care providers and patients on a wide range of health concerns including anxiety, depression, sleep, chronic pain, tobacco use, substance use, hypertension, stress management, diabetes, etc.
• Assist with administering and reviewing screening tools with patients and providers in the primary care setting to determine next steps for care including but not limited to PHQ-9, MDQ, SBIRT, GAD-7, EPDS, etc.
• Utilize motivational interviewing to assess patient's readiness for change and utilize stage matched interventions to assist with goal setting
• Partner with patients to identify barriers to medication / treatment adherence and chronic health management
• Work with the primary care team to identify care gaps and address quality metrics
• Provide culturally component care and utilize interpreting services as indicated
• Facilitate psycho-educational support groups on self-management of chronic health concerns
• Conduct brief crisis interventions and safety planning as needed
• Maintain client records, following agency and state regulations
• Maintain agreed upon level of productivity
• Demonstrate strong time management skills managing both scheduled and unscheduled appointments ("warm hand off")
