Job Description
The Case Manager coordinates all services necessary to maximize a person served recovery and independence for individuals with behavioral health needs.
In a Hospital Diversion Program, provide clinical and treatment plan consultation to direct care work force member. Ensures case management services are in compliance with agency standards and the practice model.
Assists person served to improve quality of life and maintain the highest possible ability to recover and function within the community.
Implements strategies for obtaining services and supports that are goal-oriented and individualized. This position promotes recovery-based, person-centered, and culturally competent practices.

How you will change lives
Case Management
- Ensure consumer involvement in the Person Centered Planning Process (PCP).
- Facilitate the implementation of Individual Plan of Service.
- Perform comprehensive, ongoing psychosocial assessments.
- Participate in legal proceedings including petitioning and testifying in clinical emergencies.
- Provide community based services.
- Facilitate the coordination of consumer intakes, discharges, and transfers.
- Communicate with all service providers in Treatment Plan.
- Facilitate hospitalizations, attend hospital meetings and coordinate with hospital social worker.
- Coordinate and monitor activities such as a job, school, volunteerism, and family caretaking.
- Provide referral to appropriate community resources.

We are Hopeful you have
- Bachelor’s degree in a human services field from an accredited institution required.
- LLBSW, LBSW, LLMSW, LMSW preferred.
- Prior case management experience in a mental health or rehabilitation setting highly preferred.
- Valid Michigan Driver’s License.
Every other weekend (Saturday and Sunday) - shift is flexible
