Job Description :

Position Summary : The Clinical Pharmacy Specialist Population Health works collaboratively with other health care providers to develop and implement evidence-based pharmacotherapy programs for high-risk patient care management and cost-effective prescribing initiatives for patients within the Lahey Clinical Performance Network (LCPN).

The scope and nature of patient care services are focused on optimizing patients’ therapeutic outcomes, and includes active involvement in interdisciplinary care activities, pharmacotherapy consultation, medication therapy monitoring, provision of drug information, education to both health care providers and patients, and guideline development and implementation.

Additionally, the Clinical Pharmacy Specialist supports the safe, effective and efficient use of medication through Pharmacy and Therapeutic Committee involvement and the development of prescribing guidelines, provider educational programs and network-wide communications.

Essential Duties & Responsibilities including but not limited to :

Patient Care Activities :

1. Collaborate with other members of the health care team to efficiently coordinate patient care

2. Complete comprehensive medication histories and medication reconciliation

3. Review and assess medication regimens with focus on safety, efficacy, adherence and cost

4. Adjust medications as permitted through collaborative practice agreement, where applicable

5. Screen for drug interactions, therapeutic duplications, omissions, etc.

6. Monitor patients for therapeutic and / or adverse response to medication

7. Provide medication counseling to patients and caregivers

8. Proactively assess medication adherence, and refer patients to patient assistance programs as needed

9. Ensure continuity of care and access with all medications

10. Perform home visits for high-risk / high-priority patients when applicable

11. Assist in identifying and developing interventions for LCPN patients that are not meeting target quality measures.

12. Serve as a resource to LCPN providers and care managers for general drug information and pharmacy policy.

13. Provide educational in-services on medication-related topics to patients and healthcare professionals.

Quality- and Cost-Focused Activities :

1. Participate in the development and support of formulary management and medication utilization initiatives that promote the cost-effective use of pharmacotherapy for patients through involvement in the Lahey Pharmacy and Therapeutics Committee

2. Participate in the development and implementation of evidence based treatment guidelines

3. Participate in the development and implementation of clinical and operational policies and procedures

4. Educate health care professionals about the drug therapy prescribing, management and monitoring

5. Provide support for the LCPN pharmacy newsletter through written articles and promotion of publication.

6. Develop and participate in pharmacy-related quality improvement initiatives for patients.

7. Attend all required departmental meetings as well as other meetings as assigned.

8. Promote Lahey Health through participation in professional organizations or meetings and conferences.

Educational / Administrative Activities :

1. Participate in the education and development of pharmacists, pharmacy residents and pharmacy students where applicable.

2. Participates in training of new employees and orientation of new health care providers where applicable.

3. Maintain applicable certifications, competencies and continuing education.

4. Sustain outstanding skills as a self-directed clinical pharmacy specialist and consistently demonstrate the value of pharmacy services through maintaining updated knowledge of contemporary pharmacy practice, clinical skills and health care policy.

The Anticoagulation Clinic at Lahey Hospital & Medical center offers two sites for patients : Burlington and Peabody.

Location : The primary site for this position is in Burlington with rotating shifts at the Peabody location

Schedule : Day shift; Monday through Friday; rotating Saturday shift

Hours : 3 x 10 hour weekday shifts; 1 x 4 hour weekday shift; one flex day; 4 hour shift on Saturday

Other duties as assigned.

Minimum Qualifications :

Education : Must be a graduate of an accredited college of pharmacy.

Licensure, Certification & Registration :

  • Must possess Massachusetts pharmacist license in good standing at time of hire.
  • Current Board Certification in Pharmacotherapy (BCPS), Ambulatory Care Pharmacy (BCACP) or Geriatric Pharmacy (CGP), or must obtain certification within one year of eligibility.
  • Current Basic Life Support (BLS) certification, or must obtain certification prior to performing home visits where applicable.

Experience :

  • Requires a minimum of 2 years’ experience functioning as a clinical pharmacist in an ambulatory care practice or completion of a postgraduate pharmacy residency focused in ambulatory care
  • Preferred experience in managed care principles.
  • Preferred experience in interdisciplinary Pharmacy & Therapeutics committee participation and therapy guideline development.

Skills, Knowledge & Abilities :

  • Requires analytical skills to evaluate the appropriateness of drug therapy including patient assessment, drug therapy monitoring, and pharmacotherapy principles in resolution of patient care problems related to medication therapy management and making drug therapy recommendations.
  • Requires strong literature evaluation skills and understanding of principles of evidence-based medicine.
  • Requires strong interpersonal and written skills to collaborate with all members of the healthcare team effectively to provide patient centered care
  • Must be self-directed, proactive, assertive, and possess creative problem solving skills.
  • Must be facile with physician relations, developing systems and procedures, continuous quality improvement and fiscal management.
  • Requires efficiency in use of Microsoft Office products, including Word, Excel and PowerPoint
