Details :
The Clinical Health Promotion and Education Specialist works with patients referred for assistance in treatment to prevent or reduce exposure to contracting HIV.
This position provides support to providers prescribing treatment for PrEP, and monitors patient adherence to treatment.
Clinical Health Promotion and Education Specialist will work with patients to understand and complete HIV risk assessments, as well as teach, counsel, and follow up with patients to manage identified risk behavior.
Clinical Health Promotion and Education Specialist will also support improvements in health outcomes through coordinating care, educating patients, building trust between patients and medical providers, by enhancing communication and the continuity of care.
Experience and Skills :
Education :
Associates or Bachelor's Degree in Nursing and licensure as an RN in South Carolina
Licensure / Certification :
- Must have a Licensure as an RN in South Carolina
- Must be BLS CPR certified.
Knowledge and Skills :
- Have 3 years' experience
- Experience in patient education and HIV Knowledge
- Must have working knowledge of computer programs and EHR systems
- Must demonstrate legible handwriting, excellent spelling and grammar, as well as ability to communicate clearly
- Experience working with diverse population groups
