Job Title : Case Manager
Department : Alpha Square, Permanent Supportive Housing
Reports to : Program Director
FLSA : Full-Time, Non-Exempt

Salary Range : $22.00-$25.00 an hour, commensurate with experience and education
POSITION SUMMARY
Under the direct supervision of the Program Director, the Case Manager will be responsible for providing case management to clients and will work closely with Property Management to fill vacant units through CES.
The Case Manager will work closely with community partners to schedule and maintain various onsite supportive services for the residents.
ESSENTIAL FUNCTIONS AND RESPONSIBILITIES
- Provide case management to clients;
- Work closely with Property Management and RTFH to fill vacant units through referrals from CES;
- Assist matched clients and their Case Managers with the application process, communicating all necessary information and documents that will be needed;
- Complete intakes and exits for clients upon move-in and move-out;
- Provide ongoing support and case management to ensure retention, including coordinating and scheduling appropriate client services with external medical, behavioral health, dental, legal, housing, and social / recreational activities to meet clients needs;
- Schedule onsite supportive services with various community partners and create new partnerships to bring services onsite as needed;
- Maintain appropriate documentation regarding client's progress, services received, and special incidents in individual resident case files;
- complete annual assessments as needed;
- Communicate and adhere to program policies and procedures and model professional rapport with all clients;
- Ensure compliance with organization, contractual, local / state / federal mandates, including confidentiality laws;
- Maintain a productive case management relationship with residents.
OTHER RESPONSIBILITIES :
- Work collaboratively with all team members.
- Participate in staff training, staff meetings, and development activities as directed.
- Perform other duties as directed by management.

QUALIFICATIONS

EDUCATION AND EXPERIENCE
- BA / BS in Social Work or a comparable discipline.
- 1-2 years experience in Case Management working with vulnerable populations, including those with mental illness and / or chronic medical conditions.
- The ability to communicate in person and over the phone, required.
- A firm understanding of Microsoft Office, including Outlook, Excel, and Word, required.
- Strong written and verbal communication skills, required.
- The ability to adhere to policies and procedures, required.
- Understanding confidentiality protocols and protecting personal information, required.
- The ability to organize and prioritize projects and multiple tasks effectively and on time, required.
- Effectively problem-solve and maintain composure in high-pressure situations, required.
- Act as a liaison with partner agencies.
- Ability to work independently with minimal daily supervision required.
- Knowledge of utilized practices and principles, including Housing First, Trauma-Informed Care, Conflict Resolution, Motivational Interviewing, Low Barrier Operations, and Prevention and Diversion, preferred.
- Experience with principles and practices of data collection and report preparation.
- HMIS and Clarity experience, preferred.
