Job Summary :

We are seeking a highly skilled and compassionate Clinical Pharmacist to join our team at Mission Community Hospital in Panorama City, California.

As a Clinical Pharmacist, you will be responsible for providing pharmaceutical care to our patients in coordination with other healthcare professionals.

You will also serve as a resource for drug information, educate patients about medication therapy, and monitor patient medication regimens for appropriate therapy and outcomes.

This is a full-time, individual contributor role with a competitive salary and benefits package.

Responsibilities :

  • Review and interpret physician orders for medications, including prerounding in the inpatient setting and ensuring medication appropriateness, dosages, and routes of administration.
  • Collaborate with physicians and other healthcare professionals to develop, implement, and monitor patient-specific medication treatment plans.
  • Educate patients and caregivers about medication therapy, including potential side effects and interactions.
  • Monitor the effectiveness and safety of medication therapy, and make recommendations for changes as needed.
  • Ensure appropriate use of medications through drug utilization reviews, therapeutic interchange, and formulary management.
  • Verify the accuracy of medication orders and dispensing to maintain patient safety.
  • Maintain accurate and complete patient records and medication profiles.
  • Participate in hospital committees and provide expertise in medication-related issues.
  • Adhere to all federal, state, and hospital regulations, policies, and procedures related to pharmaceutical care.

Requirements :

  • Bachelor's or Doctor of Pharmacy degree from an accredited school of pharmacy.
  • Active pharmacist license in the state of California.
  • Minimum of 2 years of experience in a clinical pharmacist role.
  • Strong clinical knowledge and critical thinking skills.
  • Excellent communication, collaboration, and patient education skills.
  • Ability to work independently and in a fast-paced environment.
  • Familiarity with electronic health record systems.
