You are a compassionate and skilled Nurse Practitioner or Physician Assistant with an active California state license and board certification.
New graduates are welcome, provided you have relevant clinical experience as a Registered Nurse (RN). You are comfortable conducting in-home assessments and have a passion for working with elderly patients.
You possess excellent administrative, organizational, and communication skills, particularly with older adults. You are detail-oriented, dependable, and able to work independently, ensuring the highest standards of care are maintained.
What You'll Do
As an Advanced Practice Provider, you will :
- Conduct in-home assessments, including past medical history, review of symptoms, physical examinations, medication reviews, and cognitive / depression screenings.
- Provide patient education on health monitoring and community resources, promoting independent and informed patient choices.
- Check vitals, BMI, and perform physical exams (excluding blood draws), formulating a list of current and past medical conditions.
- Communicate assessment findings to the primary care physician to inform potential gaps in care.
- Educate members on disease processes, medication, and compliance.
- Comply with all HIPAA regulations and maintain the security of protected health information (PHI).
- Work a schedule of 5 days in the field, seeing 4-5 patients per day, starting at 8 am, with typical shifts including approximately 2 hours for travel and 2 hours for charting.
