Practice details :

• 4-day work week and limited practice call (phone consult only)
• Medical Assistants, network of specialists, and on-site leadership to support an autonomous practice
• Clinical responsibilities include wellness and chronic care disease management, as well as specialized procedures according to candidate's interest
• Located in multi-specialty Summit View Medical Commons in west Greeley (with an incredible panoramic view of the Rockies!).

Behavioral Health, Diabetes Education, X-Ray, Lab, Pharmacy, Mammogram, PET, and more on-site

Salary Range : $235,000 - $265,000 (commensurate with experience), plus incentives

Requirements include :

• Board Certified or Board Eligible in Family Medicine
• Patient-focused, friendly and willing to collaborate in a team environment
