- Understanding of Medicare Home Care benefits and regulatory requirements
- Case management activities including assessment, planning, monitoring, and evaluation
- Completion of required documentation and timely communication with healthcare team members
- Practicing in a sensitive manner to the needs of patients and families, and directing the activities of licensed practical nurses and home health aides
- Performing OASIS assessments, complex wound care, and central line monitoring
- Communication with physicians regarding patient updates and condition changes
