During home visits, you will ensure patients are progressing with their care plan, provide education related to their disease process, and assess physical and psychological needs to improve quality of life.

When it comes time to discharge, you will coordinate resources and educate patient and patient's family.

Why Home Health

When the time comes to discharge a patient, it can be difficult because you created a special bond with them and their family and caregivers, as they allowed us the honor of coming into their homes to walk side by side with them.
