Position Summary :
As a Granite VNA Senior Director of Home Care, you will be a member of our management team and play a pivotal role in ensuring the seamless business and clinical operations of our Home Care services throughout the communities that we serve.
This dynamic position requires a candidate with proven management and clinical experience as a Registered Nurse (RN), ideally within the home care setting, who possesses the skills to lead and inspire a disciplinary team to achieve outstanding patient care and satisfaction.
The Senior Director of Home Care will drive operational initiatives and performance improvement while working to meet / exceed organizational targets.
As a result, helping the organization achieve success as the home care provider of choice within New Hampshire.
Responsibilities include but are not limited to :

- Works closely with the Vice President of Home Care and leadership team to promote operational effectiveness, meeting strategic operational goals.
- Serves as a champion communicator to ensure goals and organizational initiatives are channeled bi-directionally within the organization.
- Actively participates in Quality Improvement (QAPI) and activities, and shares outcomes and correction plan with staff.
- Has a working knowledge of third-party contract requirements in relation to clinical summary of care requirements necessary for authorization for care.
- Participates in operational improvement plans.
- Fiscally responsible for the Home Care Department’s operational success by managing budgets and performing analysis for optimal operations.
- Provides direction and guidance to all personnel in the Home Care program. RN and LPNS may receive clinical support from the Home Care Director (RN) as needed.
- Identifies any gaps in services and coverage and makes recommendations to the HP of Home Care.
- Assists the Vice President of Home Care in the development of program policies and procedures, makes revisions as necessary.
- Reviews, documents, and reports any incidents, complaints and / or grievances per agency policy and procedures.
- Works with the VP of Home Care to determine any strategic changes that are needed in order to remain a successful and competitive organization.
Other Job Functions
- Actively participates in the implementation of the agency’s Emergency Preparedness Plan.
- Demonstrates flexibility in role by accepting other duties as assigned for which qualified.
- Participates on Agency and Community Committees as necessary. Serves on Advisory Boards, committees and / or community groups.
- Participates in preparing budgets and operational plans related to home care services, and agency strategic initiatives.
- Participates in the Administrator On-Call rotation.
Will require work in all agency locations (Concord, Laconia & Wolfeboro).
Requirements
- Graduate of an accredited school of Nursing, accompanied by current New Hampshire License, or compact state license through the Nurse Licensure Compact (NLC)
- Bachelor’s Degree in Nursing; Masters in health-related field preferred.
- Five or more years of clinical experience (community health or hospital practice).
- Five to seven years of management experience.
