Columbine Commons Health and Rehab is located at 1475 Main Street in Windsor, Colorado and offers furnished private rooms designed to provide comfort for individuals requiring short-term rehabilitation following hospitalization or for long-term care, or end of life care / hospice care.
The Clinical Coordinator is responsible for the management of the medication program and the overall quality of care that is provided to the residents of Columbine Commons as well as oversight of LPNs and PCPs.
The Clinical Coordinator is also responsible to keep current on federal and state regulations for employees and residents in regard to infection control, TB, universal precautions, OSHA standards, influenza and educate as needed.
We're accepting applications for a Clinical Coordinator, LPN who can work :
- Full-Time
- Monday- Friday
- Some on-call availability
QUALIFICATIONS
- Current active Colorado Nursing license in good standing with the State Board of Nursing.
- One year of supervisory experience.
- One year of experience with geriatric population.
- Knowledge and ability to provide care coordination, assessment, and care planning to a population with chronic conditions including dementia.
- Ability to establish and maintain an effective working relationship with employees and residents.
- Ability to accept and provide supervision.
- Ability to organize and manage time efficiently and to follow written and verbal instructions.
- Understanding of quality improvement process.
Compensation
Starting wage $27.00-$35.91 / hour DOE.
