Overview
The Admissions Nurse works under the guidance of the Director of Nursing. The Admissions Nurse promotes rehabilitation guests' well-being and satisfaction by supporting person center and guest directed care.
Facilitates Admission process by collaborating with hospital discharge planners to ensure admission criteria is met for potential admission of residents.
May particIpate in assessing, planning, implementing and evaluating nursing care, and provide for the emotional and physical comfort and safety of rehabilitation guests and their families, as deemed appropriate.
What you will do
Maintains working relationships with providers and other medical personnel in support of quality clinical care in the
household. Assists medical personnel with ensuring potential guests meet the clinical criteria for the care setting
and proper equipment is identified and in place at the facility
Coordinates with discharge planners and Central Admissions regarding upcoming admissions, reviews and
completes admissions to include chart review, insurance review, face sheet, and follow-up with resident / guest after
admission
Effectively evaluates admission readiness of potential residents / guests to minimize readmissions back to hospital
environment. Participates in interdisciplinary Plan of Care with hospital.
Appropriately assesses potential admission, taking into consideration the customer's needs (services, medications,
equipment, etc.) to ensure the facility can appropriately provide efficient services
Able to complete a typical day's assigned workload within the scheduled shift. Demonstrates flexibility and offers
assistance to help team.
Qualifications
Experience
Experience in a Long Term Care or Skilled Nursing Facility (Preferred)
Licenses and Certifications
- Register Nurse (RN) - Virginia Department of Health Professions (VDHP) Upon Hire(Required) and
- CPR / BLS Certification - American Heart Association / American Red Cross / American Safety and Health Institute (AHA / ARC) within 30 Days(Required)
