- Position Summary
- Assessing, planning, implementing and evaluating, the Cardiac Rehab Health Educator is responsible for the delivery of client care : managing assigned personnel, and promoting collaboration among multi-disciplinary health care team.
The role of the Health Educator is : 1.to assess and risk stratify patients utilizing AACVPR guidelines, 2.to educate the patient and their family on interventions aimed at restoring functional capacity and effecting favorable modifications of coronary heart disease risk factors and respiratory disease,3.
to coordinate and supervise research and provide long-term patient tracking4.to promote confidence in the client' ability to achieve personal fitness and wellness goals

- Masters of Science - Required
- Experience
- 1-2 Years Clinical - Preferred
- Two or more year's experience in exercise related field
- License & Certification
- Professional Certification - Required
- National Certification - Required
- Minimum of one advanced certification : ACSM, CSCS, WellCoach, AACVPR
- Basic Life Support (BLS) - Required
- Advanced Cardiovascular Life Support (ACLS) - Required
- Core Job Functions
- Educates patients and their families on interventions aimed at restoring functional capacity and effecting favorable modifications of coronary heart disease risk factors and respiratory disease.
- Patient's blood pressure, heart rate, and physical status are monitored for abnormal responses and documented. Exercise plan is tailored to the individual needs of the patient and family;
assessment, response to exercise, and resolution of goals are documented; progress notes completed and sent to physicians.
Designs, develops, updates and evaluates educational curriculums. Accreditation standards and principles are incorporated in educational planning.
Participates on committees and serves as a content expert when applicable.
- Evaluates individual outcome measures for each client, aggregate outcome measures for the program, and the quality and outcomes.
- Nursing assessment is reviewed at the beginning of each exercise / teaching session and any abnormalities noted on exercise flow sheet and communicated to floor nurse and physician as appropriate.
Physician is informed of any critical assessment changes prior to patient being released and orders documented per policy and procedure.
Open communication with patients / families is utilized to achieve positive patient / family outcomes. Follow up on patient / family / customer complaints in a timely manner.
Attends community activities including health fairs, lectures, and visits to doctor offices.
