Providing essential personal care and supportive services to ensure the client's physical and emotional well-being, maintain a conducive home environment, and facilitate the client's journey towards greater independence.
Upholding the agency's client and family-centered philosophy by collaborating with various disciplines to deliver comprehensive and high-quality care to the client.
Essential Functions To perform the specific home health aide tasks, homemaking tasks, and delegated nursing tasks that are specified on the care plan for each individual client.
Everything that is on the care plan must be done or charted that not done and the reason not done. Nothing should be done that is not on the care plan.
If something should be added to the care plan; inform the R.N. Case Manager.Charting (as done or not done) of all tasks required by the care plan must be done.
PRN tasks should be charted if done. Do not chart anything that is not on the care plan.Report ANY change in client condition to a Baywood nurse IMMEDIATELY.
Work with client and family in a compassionate and understanding manner. Show a genuine concern for clients of all ages.
Promote a safe and comfortable environment for the client and family. Promote effective home health practices. Maintain clean, safe and healthy environment.
Support and assist clients with basic daily activities to maintain independence Read, understand, and execute the plan of care which may include : Range of motion exercises Bathing, linen change, and special care of the skin, mouth, nails, feet and hair as planned Good body alignment, positioning, and movement for non-ambulatory client Assistance in and out of bed, on and off toilet, in and out of chairs.
Never support more than 35 lbs. of the clients weight. If more is required, get assistance. Assistance with the use of bedpan, urinal, commode or bathroom Assistance with eating and dressing Reminders to take medications;
medication administration Assistance with application of prosthesis or other assistive devices Assistance with rehabilitation program as defined by RN or physical therapist Intake and Output measurement Environmental Risk : Exposure to hot and cold temperature and communicable diseases Skin contact with solvents or other chemicals such as alcohol, acetone, detergents or bleach, and latex gloves Safety Performance duties : Use transfer belt and other assistive devices as needed for the safety of employee and client.
Report unsafe client behavior to a supervisor so remedial action may be taken Report any defective equipment or environmental hazards to a supervisor Wipe up wet floors, spills, and other hazards immediately Observe special precautions as noted on the care plan for each individual client Understand and use Universal Precautions and necessary infection control measures at all times Report all incidents / accidents to a supervisor immediately Performing all other duties as assigned.
