- As a LCSW you will be reporting to the Case Coordinator, you will provide home health services to an assigned caseload; develop the Physician Plan of Care in coordination with the health care professionals assigned to the case;
- ensure that all home care services are provided in a high quality manner and in accordance with Department of Health regulations;
and support the Agency’s mission to provide the highest quality care by complying with regulatory guidelines, meeting agency standards, and obtaining appropriate certification.
Qualifications
- 2+ years of experience in Family Therapy / Counseling; 2+ years of Supervised experience in the Health field
- Master's in Social Work from a graduate School of Social Work
- Indiana Licensed Clinical Social Worker (LCSW)
- Microsoft Office / Suite proficient
- PC / IPAD proficient
- Solid assessment, clinical, and documentation skills
- Patient oriented
- Great interpersonal skills
- Excellent communication skills (written and verbal)
- Strong attention to detail
- Highly organized
Responsibilities
- Contribute to patient and family counseling and education
- Participate in ongoing interdisciplinary comprehensive assessment
- Assess patient / family psychosocial response to the illness and evaluate coping skills
- Assist patient, family and agency staff in utilizing community resources
- Assist team members in understanding the significant social and emotional factors related to the care of the patient and family
- Coordinate with team members to plan and implement the home care plan of care
- Participate in interdisciplinary team meetings
- Maintain accurate, thorough and professional written records, and charts relevant information in a timely fashion
- Develop and implement the discharge plan
Compensation
- Reimbursement per visit and mileage
- PRN Position with great flexibility
