Primary Health Care (PHC) was founded in 1981 by Dr. Bery Engebretsen in Des Moines, IA. Our mission has remained unchanged since that time, to provide healthcare and supportive services to all, regardless of insurance, immigration status, or ability to pay.
Based on the needs of the communities we serve, PHC offers a spectrum of medical and dental services including family practice, behavioral health, HIV care and services, and pharmacy.
PHC’s Homeless Support Services is the entry point for serving people experiencing homelessness in Polk County. Enabling services are available to help patients with benefits enrollment, case management, transportation, translation, and patient education.
We currently have locations in Ames, Des Moines, & Marshalltown.
As the Grant Program Coordinator, you will implement and coordinate grant deliverables, data tracking, and reporting for assigned grants.
This position is funded by a HRSA Maternal Health Grant for a two-year period.

What You Will Do
Executes program goals ensuring alignment with organizational strategic plan. Organizes, writes, and submits grant progress reports within required time frames.
Implements and oversees project activities, work plan, and data collection for assigned grants. Monitors clinical outcomes of clients and / or patients participating in the program including health disparity and quality improvement initiatives.
Collaborates with Grant Project Officer for technical assistance regarding report guidelines and requirements. Maintains up-to-date knowledge of the rules and deliverables of the grant.
Develops and maintains community partner relationships through proactive communication and participation in workgroups and meetings.
Communicates and manages grant partnerships and stakeholder expectations. Organizes and facilitates grant steering committee meetings to monitor the goals of the program while ensuring completion of action items and maintaining meeting minutes.
Distributes actionable data in accordance with the grant data collection plan. Monitors and presents data and progress updates in writing and verbally to the staff members and accountable community partners.
Monitors grant revenue and expenses, develops program budget, and makes fiscally responsible decisions. Attends monthly budget meetings and monitors monthly billing statements.
Approves daily expenditures maintaining costs within monthly budget. Assists in writing grants for programs within the department.
Ensures compliance with grant and regulatory requirements through quality assurance of documentation and data fidelity.
Maintains coordination of technology applications to ensure appropriate documentation and data entry for all areas of the client and / or patient record and grant requirements.
Maintains compliance with Primary Health Care policies and procedures and with external regulatory bodies.
Develops and facilitates departmental performance improvement initiatives to ensure effective and efficient workflows.
Facilitates data driven performance improvement initiatives to assist with the development of efficient workflows, client satisfaction, staff satisfaction and cost-effective interventions.
Collaborates with grant leadership and the Homeless Support Services Director to develop quality assurance tools used to measure staff performance and documentation oversight.
Performs other duties as assigned.
Qualifications You Need to Bring
Required
- Bachelor’s degree in psychology, counseling, mental health, human services, or other related field or equivalent combination of education and experience (if allowed by granting agency).
- Minimum 2 years of relevant work experience in human services.
- Experience using client / patient databases and learning other software applications.
- Experience working under and complying to policies and procedures that include safety, attendance, punctuality, appearance and other standards.
- Proficiency with Microsoft office applications including Excel and other computer functions.
- Organization, prioritization and time management skills with professional experience.
- Strong interpersonal and relationship building skills with ability to demonstrate caring, respect, ethical and empowering behavior towards others.
- Team orientation and collaboration skills.
- Strong verbal and written communication skills.
- Customer service orientation with high commitment to excellence.

Preferred
- Experience working in a healthcare environment utilizing medical terminology specifically in the area of maternal health.
- Experience working in or with a community health center or rural health center.
- Experience working in or with non-profit agencies and / or grant funded environments.
We Take Care of Our People
Your experience and skills determine your base pay. The hiring range for this position is typically $42,800 - $53,500 per year though highly experienced candidates may earn up to $64,200 per year.
