What This Role Entails :
Are you ready to make a meaningful impact while having fun? Join us as a Case Manager! In this exciting role, you'll be the go-to person for enrolling participants into our dynamic program.
You'll assess and address their emotional, social, and environmental needs, providing them with vital casework services like information, referral, psychosocial support, and advocacy.
You'll work closely with a vibrant team of healthcare, mental health, and social service providers, all dedicated to helping participants thrive and fully engage in the program.
Plus, you'll get to flex your organizational skills by assisting with administrative tasks, including preparing reports and tracking program activities.

Required Minimum Knowledge, Skills, and Abilities (KSAs) What you will need :
- Education : Bachelor’s degree preferred or equivalent, relevant experience in a healthcare environment.
- License / Certifications : May be required to travel off-site. Will need valid state driver's license and reliable vehicle.
- Experience : Approximately two years on the job experience in a community health setting in order to consolidate and demonstrate knowledge of community resources and skills in assisting people to make decisions about their health and health care.
Ability to ensure that a wide variety of other disciplines or community resources work cooperatively in order to maximize the health and well being of the enrollee / participant.
