Community Health Worker
HealthCare Support is actively seeking a Community Health Worker to fill an opening with a Managed Care Organization in the State of Florida .
Daily Responsibilities for Community Health Worker :
Responsible for delivering of a range of activities for individuals who are enrolled in the health plan for Medicaid or and Medicare in order to impact individual health outcomes and provide assistance to the clinical team of nurses and social workers.
Activities include, but are not limited to outreach, community education, informal guidance, and member support.
- Complete field visits assigned (80 field visits per month with minimum of 20% successful visits - assessment or screening completed).
- Make outreach calls from reports.
- D ocument using TruCare.
- Attend training as needed.
- This will be a Contract position, schedule is Monday to Friday 8AM to 5PM Eastern Time, and target start date is 07 / 08 / 2024.
Required Qualifications for Community Health Worker :
- High School diploma or GED is required for the position
- 2+ years of community health, social work, social services, community advocacy, community outreach, member services, or member education experience
- Must have experience completing Home Visits
- Understanding of the community in Palm Beach County through shared experiences or a strong desire to help people in vulnerable communities.
- Proficiency in MS applications
- Must have a valid and active driver's license.
- WFH capability smart phone, private workspace, high speed internet
Preferred Qualifications for Community Health Worker :
- Community Health Worker Training / Certification
- Bilingual in English and Spanish
