Key ResponsibilitiesEngage members telephonically and schedule consultation appointmentsUtilize prescription claims data to generate clinical recommendations and assess medication regimensProvide patient education on medications and accurately document care notes and recommendationsRequired QualificationsBachelor's Degree in Pharmacy or PharmDCurrent and unrestricted Pharmacist license in LouisianaStrong knowledge of disease state management guidelines and MTM servicesExperience working remotely and independently within a cross-functional teamAbility to navigate approved MTM platforms and utilize multiple databases
