The role of the RN-CaseManager supports our organization by ensuring patients aredischarged from the hospital safely, identifying any barriers andworking through those with the patient and careteam.

JobSummary :
Assesses, develops, plans,implements, and evaluates patients discharge planning needs to nextlevel of care in collaboration with interdisciplinaryteam.
Job Responsibilities andRequirements :
PRIMARYRESPONSIBILITIES
- Ensures compliance withCondition of Participation (CoP) dischargeplanning.
- Identifies and escalatesgaps / barriers related to inpatient and observation length ofstay.
- Assesses patients at high risk forreadmission. Identifies root cause for readmission. Develops planand collaborates with multidisciplinary team regarding dischargeplanning to next level of care.
- Participateswith other members of team regarding opportunities for improvementin standard work.
- May also be responsible for : utilization management process, ED case management process,admission process, and act as a case manager liaison with postacute providers (skilled nursing facilities, rehab, home health,etc.).
- Applies the existing body ofevidence-based practice and scientific knowledge in health care tonursing practice, ensuring that nursing care is delivered based onpatient's age-specific needs and clinical needs as described in thedepartment's Scope of Service.
- As an SSM Healthnurse, I will demonstrate the professional nursing standardsdefined in the professional practice model.
Uses the ANA Code ofEthics for Nurses to guide his / her response to the current andevolving health and nursing needs of our patients and our patientpopulations.
- Works in a constant state ofalertness and safe manner.
- Performs otherduties asassigned.
EDUCATION
Graduateof accredited school of nursing or education equivalency forlicensing
EXPERIENCE
Threeyears' acute care experience
PHYSICALREQUIREMENTS
- Frequent lifting / carrying andpushing / pulling objects weighing 0-25lbs.
- Frequent sitting, standing, walking,reaching and repetitive foot / leg and hand / armmovements.
- Frequent use of vision and depthperception for distances near (20 inches or less) and far (20 feetor more) and to identify and distinguishcolors.
- Frequent use of hearing and speech toshare information through oral communication. Ability to hearalarms, malfunctioning machinery, etc.
- Frequentkeyboard use / data entry.
- Occasional bending,stooping, kneeling, squatting, twisting andgripping.
- Occasional lifting / carrying andpushing / pulling objects weighing 25-50lbs.
- Rare climbing.
