Active UT License or IMLC
BC in OB / GYN (ABOG)
DEA (If transferred DEA : must keep for the duration of the assignment not just for shifts worked)
Must be available for all functions of the job description.
Clean record or Max of $100k total in MP claims & with Self Inquiry NPDB report.
