state, federal, and local laws; and Nurse Practice Act. This is a salary exempt position.QUALIFICATIONSGraduate of an accredited school of professional nursing.

BSN preferred.Current license as a Registered Nurse in the state(s) of practice.Minimum of one (1) year nursing experience in an acute hospital setting or equivalent experience with one year of supervisory experience.

Complies with accepted professional standards and practice.Excellent verbal and written communication skills and strong interpersonal skills.

Knowledge of documentation requirements within home health care.Proof of driver' s license and automobile insurance in accordance with Agency requirements.

Has current CPR certification.ESSENTIAL FUNCTIONS / AREAS OF ACCOUNTABILITY1. Reviews client referral information and responds to requests and inquiries as appropriate2.

Performs initial and ongoing client assessments based on Agency policy and standards of practice to ensure effective and appropriate home care services.

3. Considers the physical, psychological, and socioeconomic needs of the client, the level of family acceptance, and the ability for client needs to be met in the home.

4. Collaborates with physicians, other health care professionals (therapists, social services, pastoral care, supportive services), client s, and families in developing a comprehensive, coordinated plan for care.

5. Directs, plans, and initiates appropriate action independently and responsively inhome care situations.6. Monitors documentation of clinical records to ensure compliance with regulatory standards of timeliness, accuracy, and completeness.

7. Provides on-call support for clients per schedule.8. Communicates with other disciplines / departments when required.9.

Manages / supervises a team of RNs, LPNs, Home Health Aides, and Persona l Care Aides to provide effective and quality home care services.

10. Identifies the educational needs of team members. Provides training or refers to clinical supervisor to arrange specific training program.

Provides or recommends educational resources.11. Performs or delegates in-home supervision of team members.12. Conducts timely performance evaluations consistent with Agency policy.

Conducts in-home supervision of staff per Agency policy, regulatory requirements, and as needed.13. Promotes personal safety and a safe environment for clients and coworkers.

14. Recognizes and responds appropriately to potentially unsafe situations.15. Participates in all mandatory education per established guidelines.

16. Performs job duties in accordance with Agency policies, procedures, and professional and community standards.17. Maintains confidentiality in all aspects of the job.

Does not reveal information from client records to others, except as identified in Agency policy.18. Performs other job duties as assigned.

19. The ability to frequently sit, stand, walk, bend or squat20. The ability to occasionally crawl, climb, or reach above shoulder level21.

The ability to occasionally lift up to and over 50lbs.22. The ability to continuously see, hear, speak, and use both hands
