University Home Care is currently seeking a Physical Therapist to join our team in the Ann Arbor area.
As a Physical Therapist, you will be responsible for providing direct patient care, conducting evaluations, and developing customized treatment plans for patients in the Ann Arbor area.
Your responsibilities will include :
- Evaluating and assessing patients' conditions and needs.
- Developing and implementing individualized treatment plans.
- Monitoring patients' progress and making adjustments to treatment plans when necessary.
- Collaborating with other healthcare professionals and interdisciplinary teams.
- Providing education and guidance to patients and their families.
- Maintaining accurate and up-to-date patient records.
To be successful in this role, you should have :
- A valid Physical Therapist license in the state of Michigan.
- A minimum of 2 years of experience as a Physical Therapist.
- Strong interpersonal and communication skills.
- The ability to work independently and as part of a team.
