Department : Infectious Disease / Healthcare for the Homeless
Reports To : Director of Infectious Disease / Director of Homeless Services
Responsible for direct patient care to persons experiencing unsheltered homelessness by completing street outreach (going to patients’ encampments), as well as other areas services PEH including but not limited to homeless day time / drop in centers, and soup kitchens.
Knowledgeable of HIV treatment and prevention, HCV treatment and prevention, SUD, MOUD, MAT, and primary care.
Essential Duties and Responsibilities :
- Provides comprehensive medical evaluation, education and treatment.
- Maintains patient confidentiality at all times and ensures to follow agency, state and HIPAA laws.
- Assesses and interprets medical and psycho-social histories; performs physical examinations; identifies problems; formulates a plan of treatment;
provides education and referral (as indicated); and evaluates the effectiveness of the interventions prescribed.
- Performs or orders laboratory tests and interprets the results as part of the health management. Reports communicable disease in a timely manner to the appropriate Health Department as per health center protocol.
- Orders appropriate medication.
- Implements standardized evaluations or assessments in conjunction with multidisciplinary team as appropriate.
- Ensures required documentation is complete, timely and accurate at all times.
- Obtains and tracks proper insurance authorizations as needed to provide services.
- Understands the medical emergency procedures and is familiar with the location of the emergency kit / supplies in all clinic settings.
- Participates in continuing medical education activities to enhance professionalism and to ensure quality of care.
- Provides on-call coverage as scheduled.
- Establishes and maintains good working relationships with community agencies.
- Attends and participates in team conferences / huddles.
- Attends supervision meetings as required.
- Performs all other duties as required.
Qualifications :
- Master’s Degree in Nursing. Must hold valid a CT APRN and RN license.
- Minimum of 2-4 years experience working with relevant population required including managing HIV / AIDs treatment and prevention.
- Knowledge / Abilities : American Academy of HIV Specialist preferred. Suboxone DATA X waiver preferred. Knowledge and experience working with patients with substance use disorder and appropriate management.
Competency regarding diagnosis, assessment and treatment of HIV, HCV, STIs. Ability to work with interdisciplinary team.
Ability to generate high levels of client satisfaction. Must demonstrate age specific / population specific competencies in area(s) of practice.
Knowledge and skills related to individual, group and family system dynamics. Must demonstrate cultural competence. Maintain CMEs / CEUs required for licensure.
Maintain DEA registration. Certified by ANCC or AANP. CPR certification required.
