Job Description : Medical Liaison
Primary Work Location : Harc, Inc. Residential Programs
- OCCUPATIONAL DESCRIPTION
- Assist Program Registered Nurse (RN) and Director of Nursing (DON) in the following areas :
- Provide oversight and maintain records pertaining to the DDS Medication Administration
- Review routine and / or specific responsibilities to Med Cert Staff.
- Be familiar with DDS & agency policies and procedures.
- Support group home staff in areas that pertain to the individuals supported by Harc, Inc. including, but not limited to, medication administration, universal precautions, OSHA kits, medication precautions and all associated documentation requirements.
- Support the individuals by assisting them with the following :
- Tracking, monitoring, and scheduling medical appointments.
- Ensure accurate transcription of doctor’s orders and communicating directly with the pharmacy and group home staff to ensure proper transcription of MD orders.
- Monitor medication, medication MAR’s / Kardex daily, medication errors and MD orders.
- Communicate information regarding upcoming or scheduled appointments to the Program RN and / or DON, medical providers, staff, and families.
- Assist Program RN and / or DON in maintaining quarterly med audits, monthly Kardex reviews and physician’s orders, and quarterly nursing reviews when necessary.
- Document all relevant information on Therap.
- Keep in close communication with the Program RN, Program Manager, and / or DON i.e. med errors, tracking and maintaining error reports, etc.
- Complete monthly Med Room Closet Audits including the destruction of expired medications with Program RN / DON supervision, in compliance with relevant legal requirements and the organization’s policies and procedures.
- Provide direct support coverage to individuals in lieu of staff or when otherwise deemed necessary by the Program RN and / or DON, including but not limited to, attending medical appointments (residential only), administering medications, providing personal care, etc.
- Complete and maintain all training required through the new employee orientation process including CPR, Bloodborne Pathogens and any other ongoing training that may be required to fully support the residents.
- Assist in tracking CPR certifications, delegations, and med certification documents in the group homes, to ensure they are up-to-date.
- Monitor, report and document circumstances occurring at the time a person experiences a seizure, injury, illness, behavioral episode, and / or any other medical or emotional event.
This information will be reported to the Program RN, Director of Nursing and appropriate program management staff.
Support the Program RN / DON and Program Manager by being an active participant during Planning and Support Team Meetings.
Participation includes, but is not limited to; attending team meetings, helping to assure medical progress data is being reported / recorded or obtained on a consistent basis.
