Silver Lake Hospital of Newark, NJ is seeking to hire an experienced full-time Clinical Liaison to serve as the direct contact for patient referrals in the following Pennsylvania and New Jersey Counties : Philadelphia County, Chester County, Delaware County, Sussex County, and Warren County to our long term care facility.

Silver Lake Hospital is a long-term acute care hospital with a reputation of providing patients with a place to heal before heading home.
A long-term acute care hospital (LTACH) is an acute care specialty hospital that provides highly advanced and focused care for higher acuity patients who have not responded to short-term treatment in traditional general hospitals.
The average length of stay in an LTACH is 25 days as compared to a 3-5 day average stay in a general hospital. Our safe and state of the art hospital is designed to admit and care for the most critically ill patients.
POSITION SUMMARY
The Clinical Liaison serves as the direct contact between the patient, physician, and referring facility to ensure a seamless admissions process.
The admissions process includes assessing the clinical status of patient referrals and evaluating patient needs; performing insurance verifications to obtain authorization for approval;
assisting the referring and accepting institutions in addressing needs of specific patients.
This position requires traveling in your vehicle to the referring facilities. Reimbursement for travel and other expenses is provided.
QUALIFICATIONS
- Experience facilitating admissions from referring facilities / physicians.
- Experience with critical care, trauma, wound care, or other specialty settings.
- Knowledge of Medicare and insurance verification.
- Superior communication skills with all levels of hospital staff, physicians, and patient referral sources.
- Proficient in Microsoft Office.
