Job Objective : Assist clients in finding community resources and services to meet their identified needs as they move towards self-sufficiency.
Essential Functions :
- Conduct interviews with potential clients to determine the level of need
- Provide assessment of client's psychological, emotional, medical, financial, spiritual and substance abuse status
- Complete all necessary paperwork and submit to program manager for weekly review
- Complete client's orientation paperwork within 48 hours of admission to show that they have been introduced to all program rules, policies and client rights
- Attend weekly house meetings and facilitate when needed
- Monitor client activities to provide a safe environment that is substance abuse and alcohol free
- Address appropriate program problems as needs arise, informing chain of command of same
- Document all client sessions and client incidents, and submit to program manager
- Maintain individual client case files as assigned, documenting client progress by end of work day
- Provide transportation to clients going to various appointments as needed
- Provide reports and statistical information as directed
- Be aware of current trends and resources available in order to provide comprehensive services and referrals for clients
Minimum Qualifications :
Education : Minimum Bachelor degree in Social Work from an acc redited university or a related human service field of study ;
relevant experience in lieu of education requirement will be considered.
Experience : One year of case management experience
Certifications / Licenses : First Aid and CPR certification; if not certified, must be able to obtain certification within 90 days of hire date

- Ability to maintain appropriate confidentiality including 42 C.F.R. Part 2 and HIPPA standards and procedures
- Good interpersonal skills
- Ability to relate positively and maintain professionalism through leadership and example, while establishing positive relationships with a diverse population

- Ability to use sound judgment and reasoning
- Ability to use Microsoft Office software programs

Physical Requirements : Include s pea king, hearing and vision; standing, bending, squatting and walking; sitting for extended periods of time;
manual dexterity ; l ifting, pulling and pushing materials up to 50 lbs.

Working Conditions : Work is performed in a typical office environment; m ay require some week end and evening work
