Job Summary

Provide assessments and therapeutic interventions which meet the psychosocial and mental health related needs of adult oncology patients and / or family members.

This role will provide support to the interdisciplinary team of the MMCC by developing and implementing patient-centered multidisciplinary case plans.

This position will serve as a community liaison to provide education and build partnerships resulting in improved community health outcomes. Job Responsibilities

  • Complete Psychosocial Assessment of Oncology Patients
  • Provide therapeutic intervention to individuals and / or families.
  • Systematically and continually assesses the patient's physical and mental condition in collaboration with the patient / family.
  • Identifies and refers patients to appropriate community resources as needed.
  • Facilitate Oncology Support Groups.
  • Provides educational sources for individuals, family and community.
  • Participate in accreditations mandated activities (Tumor Boards, Conferences, Committees, Projects, Data Analysis, etc.).
  • Provide outreach to bereaved family members.
  • Develop and maintain a resource center to address basic needs of current patients
  • Develop / Assist with special events (Survivorship, Bereavement, Screening, Foundation, etc.)
  • Provide supervision to undergraduate social work students
  • Fulfill mandatory Psychosocial Coordinator position for Oncology's accreditations
  • Work with providers for proper hand off to appropriate community resources (Psychiatrists, Palliative Care, Hospice, etc.).

Qualifications

  • Master's degree or higher in Psychology required upon hire OR
  • Master's degree or higher in Clinical Social Work required upon hire OR
  • Master's degree or higher in related field required upon hire
  • A minimum of 2 years' relevant experience required
  • CSW - Certified Social Worker in the State of Kentucky required upon hire AND
  • BLS - Certified as a Basic Life Support Healthcare Provider required within 90 days of employment in this position AND
  • LCSW - Licensed Clinical Social Worker in the State of Kentucky required within 36 months of employment in this position
