The Physical Therapist is responsible for performing patient evaluations, as well as providing and documenting therapy services in accordance with the plan of care developed for each individual patient and physician’s orders when applicable.
Essential Duties and Responsibilities : Performs initial comprehensive assessment and reassessments and regularly reevaluates patient for rehabilitation needs, and works in collaboration with the physician and other members of the healthcare team to modify the plan of care as appropriateParticipates in developing the plan of care in collaboration with the physician and other members of the health care teamAssesses and evaluates and / or assists the physician in assessing and evaluating therapeutic, rehabilitative, function status, and home environment (as applicable)Assesses muscle strength, mobility, gait, and Range of Motion (ROM)On incisions resulting from orthopedic surgery, removes staples or sutures and manages incision lineProvides physical therapy treatment and establishes physical therapy plan of careInstructs patient, families, and / or caregivers in the use of and care of therapeutic appliancesDetermines priority needs for physical therapyReports patients’ reaction(s) to treatment or changes in condition to physician and / or designeeInitiates physical therapy program and instructs, consults, and / or advises other personnel and / or family / caregiver regarding certain phases of physical therapy with which they may work with the patient, and explains the goals of the physical therapy program by participating in case conferencesTrains patient in use of prosthetic device(s) and use of other assistive devices (e.
g., walker); ensures equipment used in the course of patient treatment is in good working orderUses appropriate equipment to meet patient goals, cleans and maintains equipment according to manufactures guidelinesPrepares and submits clinical and progress summaries based on the attainment and progress of goals, and responses to treatment or changes in conditionParticipates in discharge planningParticipates in peer consultation processSupervises Physical Therapy Assistants according to the company’s policies / procedures and state regulationsProvides / submits accurate and timely documentation in accordance with the plan of care and company policy and proceduresComplies with applicable local, state, and federal laws and regulations regarding professional standards and practice standardsPerforms other duties as assigned / necessaryMinimum Requirements : Current licensure as a Physical Therapist in the state of practiceGraduate of an accredited school of Physical TherapyOne (1) year of prior professional Physical Therapy experience preferredComplies with all relevant professional standards of practiceParticipation and completion of Amergis' Competency program when applicableCurrent CPR if applicableTB Questionnaire, PPD or chest x-ray if applicableCurrent Health certificate (per contract or state regulation)Must meet all federal, state and local requirementsSuccessful completion of new hire training as applicable to job siteUnderstand patient confidentiality and HIPAA requirementsAbility to effectively elicit / provide information to and from appropriate individuals (including, but not limited to, supervisors, co-workers, clients) via strong communication skills;
