Overview
The Virtual Case Manager manages a caseload of patients by leveraging technology to provide the best care possible and develop a well rounded plan of care that follows all regulatory standards.
Perform Remote and / or phone visits for patients regularly. Collaborate daily with all staff involved in the patient’s care and document collaboration appropriately.

2. Adheres to the Corporate Compliance Program, including confidentiality of HIPAA protected health information.
3. Establishes a care plan for continued needs based on the patient assessment and provides care virtually following the plan of treatment and patient care plan.
4. Evaluates and revises care plans based on changes in patient needs.
5. Reports changes in patient condition to appropriate personnel and the attending physician, as required and participates in coordination of services with other disciplines and community resources as needed.
6. Facilitates case management by collaborating with physicians and multidisciplinary team members; providing support to patients and their caregivers.
7. Demonstrates sensitivity to the needs, customs, or feelings of patients and caregivers.
8. Acts as a clinical resource for field clinicians, patients, caregivers and the organization, as well as other external customers.
Provides patient care education to patients and other caregivers.
9. Participates in case conferences, summary reports, quarterly record review, annual program review and in-services or special case orientations as needed.
10. Completes and submits all required documentation in an accurate and timely manner.
11. Supports patient self-management and improved outcomes through coaching via remote visits.
12. All other assigned duties as assigned.
Qualifications
Graduate of an accredited school of nursing. Licensed to practice in the state serviced, including compact nursing state licensure.
Must obtain and maintain current CPR or BLS Certification. Compliant with appropriate state certification / registration and company requirements, as necessary.
One year RN nursing experience or three years LPN / LVN nursing experience providing direct patient care in a home care setting.
Areas of specialization require related experience.
Demonstrate RN nursing competencies : clinical skills, critical thinking abilities, OASIS,infection control, physiological knowledge, administering medication,medical teamwork, multi-tasking, effective communication, health promotion and maintenance, case management (as applicable), and documentation.
Able to problem solve effectively. Capable of prioritizing and handling multiple tasks of critical importance simultaneously.
Able to function well in an atmosphere of stress and interruption. Competent with computers; organized; detail-oriented;
effective oral and written communication skills.
Two years of hospital / hospice / homecare RN nursing experience preferred.
