The RN Case Managers overall responsibility is to provide direct patient care and case management to an assigned caseload in the home setting in accordance with physicians orders.
This includes admission of patients for service and determining and developing a plan of care.
Job Responsibilities :
- Provides direct patient care and case management. Competently implements 24-hour responsibility for each patient including nursing and home health aide service.
- Identifies and documents patient and family needs through comprehensive assessment. Reports all findings to M.D., coordinates services, and follows through on Physician orders.
- Performs admission procedure as per RVNAhealth policies using legible, accurate documentation. Develops a plan of care consistent with the physicians plan of treatment.
- Performs evaluation of patients progress, promptly acts on any changes made to patients plan of treatment or condition, including appropriate discharge procedure when goals have been met.
- Updates and maintains the nursing care plan. Documents communication and interprets plan of care to the patient regarding diagnosis and treatment.
Qualifications / Expectations
- Graduation from an accredited School of Nursing.
- Currently licensed to practice as an RN in the State of Connecticut.
- Has a minimum of one year of nursing experience.
- Ability and willingness to follow Agency policies and procedures and to work as a team member supporting the agency mission, vision, and values.
