This field-based role with a home-based office allows for flexibility in both field work and telecommuting. As part of our team, you will have the opportunity to assess, plan, and implement care strategies for Medicare and Medicaid members, utilizing strong skill sets, managing transitions of care, and proactively engaging members to manage their own health and healthcare.

Primary Responsibilities :
- Assess and implement individualized care strategies for Medicare and Medicaid members
- Engage members to manage their own health using Motivational Interviewing Skills
- Manage transitions of care for members discharged from in-patient hospital stays
- Support members to achieve health goals and improve well-being
- Identify and initiate referrals for social service programs
- Provide clinical support for non-clinical staff on the team
- Collaborate with interdisciplinary team to ensure appropriate services
- Strong computer and software navigation skills are critical
Required Qualifications :
- Current, unrestricted RN license
- 2+ years of community nursing experience
- Intermediate level of proficiency with MS Word, Excel, and Outlook
- Ability to travel up to 75% of the time with reliable transportation
Preferred Qualifications :
- Bachelor's degree in nursing
- Experience in managing Medicaid and Medicare populations
- Experience with electronic charting
