We are currently looking for qualified candidates to fill a part-time LPN position at our Perrysburg, OH office. Ideal candidates must have an active RN or LPN license issued by the state of Ohio.
Must be able to travel to various locations as needed and have schedule flexibility.The Licensed Practical Nurse works under the supervision of the Nurse Manager or Chief Medical Officer and is responsible for monitoring the physical health of persons served, providing nursing services, witnessing urine screens, monitoring OARRS reports as clinically indicated, communicating with other health care practitioners regarding client health status and providing consultation to other staff regarding health care issues.
This is an hourly position with hours and schedule established to complete the required duties. This position is not exempt from wage and hour regulations as defined by the U.
S. Department of Labor and is therefore granted protections under the Wage and Hour Division of the Fair Labor Standards Act.
EDUCATION / EXPERIENCE / LICENSE / CERTIFICATIONRequired* Active RN or LPN license issued by the state of Ohio* Two years of nursing experiencePreferred* Associate's or Bachelor's Degree in nursing or a medical related field from an accredited college or university* Experience in a psychiatric or substance use treatment settingESSENTIAL DUTIES AND RESPONSIBILITIES* In collaboration with the team, ensure a comprehensive health history is obtained and appropriate referrals are made, as needed, for further assessment and treatment.
- Provide education to persons served, family members of persons served, and other staff members regarding medication, symptom management, wellness, and health.
- Provide ongoing assessment of persons served's mental illness or substance abuse and physical symptoms as well as their response to treatment;
communicate these changes to the treatment team and make appropriate changes in treatment plans to ensure immediate and appropriate interventions are provided.
- Review Health History Questionnaires and follow up with clients as appropriate.* Provide direct clinical services to persons served to monitor symptoms, carry out prescriber orders, and teach behavioral symptom-management techniques to alleviate and manage symptoms.
- Witness urine screens and process appropriately.* Perform duties in accordance with applicable professional ethics and standards and established A Renewed Mind policies.
- Comply with all state and federal laws regarding the protection of protected health information.* Maintain complete and current medical records regarding all persons served.
