EFFC & SFFC Program Description
Catholic Guardian Services' Enhanced Family Foster Care (EFFC) and Specialized Family Foster Care
Programs (SFFC) provide family permanency services to children and families served within the
program. Our program teams are interdisciplinary and are composed of Case Planners, Social Work
Supervisors, Directors of Socio Services, Parent Advocates, Case Aides and Socio-Therapists.
Additionally, our middle school, high school and college students receive academic, employment
readiness and housing support from our Journey Program (Fair Futures).
Our work includes the implementation of evidence-based models to address trauma, mental health and
other challenges faced by children and families. We successfully provide wrap-around services and
support during and after foster care placement by having direct access to Catholic Guardian
Services' portfolio of social services and behavioral health programs. These programs include
children's health home care management services, evidence-based preventive services programs,
outpatient mental health services, career development and job readiness services for adolescent.
Our Enhanced and Specialized Family Foster Care Teams closely collaborate with these programs to
ensure ongoing support to our children and families.
POSITION DESCRIPTION :
The Adoption / KinGap Case Aide is responsible for ensuring the permanency of assigned Adoption and
KinGap related cases. The Case Aide works directly with pre-adoptive foster families, kinship
foster families, case planning staff, ACS and the courts to coordinate and expedite permanency via
adoption and KinGap finalizations.
The Adoption / KinGap Case Aide is responsible for representing Catholic Guardian
Services in a professional and responsible manner at all times
MAJOR DUTIES & RESPONSIBILITIES :
Provides exceptional customer service to pre-adoptive and kinship foster parents by assisting
them to gather and complete needed adoption and KinGap documentation leading.
Responsible for the facilitation and completion of the KinGap packets including, the
application, expense agreement, affidavit of relations, and supporting documentation.
Collaborates with the agency's adoption subsidy point person and the Special / Exceptional Rate
Liaison.
- Ensures that necessary paperwork required to finalize the Adoption and / or KinGap is obtained.
- Works directly with Case Planning staff and liaises directly with ACS KinGap Unit to
troubleshoot any existing barriers to finalization.
Educates foster families and staff (through trainings / in-services, meetings, etc.) on the
nuances of Adoption and KinGap forms.
Secures Remand and Permanency Hearing Orders for all children placed in kinship foster homes.
Electronically stores the orders for future use in the processing of KinGap applications.
Prepares reports for assigned case planning team updating them on the status of adoption and
KinGap documents.
- Documents all case activities in CONNECTIONS.
- As needed, participates in Interdisciplinary Team Meetings (ITM), Family Team Conferences
FTCs), court hearings as required by family court, and other meetings as needed / required.
- Attend weekly Freed Child / KinGap telephone conferences.
- Attends and completes in-person and virtual trainings facilitated by the agency, ACS, and other
outside organizations.
- Is prepared and actively participates in individual and group supervision sessions.
- Performs all other related duties as assigned.
QUALIFICATIONS & COMPETENCIES :
- High School or Associate's Degree
- Fluency in English and Spanish preferred
- Must have an ability to provide excellent and compassionate customer service
- Ability to work independently and exercise a high level of confidentiality
- Ability to manage time effectively, multi-task and determine priorities
- High degree of organization and attention to detail; resourceful / solution-focused
- Experience and facility with Microsoft Office Applications
- Ability to work flexible hours as needed, including occasional weekends
- Able to travel extensively to all NYC boroughs
- Able to uphold the mission and values of the agency
