The Therapist plans, provides, coordinates, documents, and evaluates direct and indirect social services to patients in accordance with established methods and procedures of the hospital / adolescent PHP / IOP program.
These services include but are not limited to the following : individual therapy, group therapy, family therapy, case management, treatment planning, rounding with clinical staff, attending / conducting unit and hospital meeting, discharge / care planning and follow-up resource development / assistance, and serving as a member on various committees.
The Therapist further contributes to Social Services and the hospital through support of philosophy and objectives, educational efforts and Quality Management efforts.
The Social Worker / Therapist is available to work any unit / department within the hospital as needed.
