Job Description and Duties

Join the Patient Management Unit at a key moment in our history as we strengthen our clinically-driven, patient-centered preadmission pathway for DSH patients! Under the direction of the Nurse Consultant, Program Review of the Patient Management Unit and with additional license support and supervision from the Clinical Operations division, the Nurse Practitioner is responsible for participating in Care Coordination efforts and providing evaluation and consultation to aid in the appropriate placement of patients during the preadmission process or in support of specific escalated instances within facilities of the Department of State Hospitals (DSH).

The Nurse Practitioner is a critical member of PMU’s Care Teams and collaborates closely with other team members to provide exceptional service to assigned counties.

Working Conditions

This position is physically located in Sacramento and is currently offered with the potential for a hybrid work environment.

Consistent with Policy Directive 5338, Telework may be considered with prior approval from management. If approved, incumbent is expected to report to work at the Allenby building at least twice per week, as directed by their supervisor.

Travel throughout California for site visits, including overnight travel, to DSH partnered treatment facilities will be required with prior notice.

DSH-Sacramento is located at a new 11 floor high rise building.
