Role Description This is a full-time on-site role as a Case Manager at Open Arms Community Health & Service Center in Los Angeles, CA.
The Case Manager will be responsible for providing comprehensive case management services to individuals and families, including assessments, referrals, advocacy, and coordination of services.
They will collaborate with a multidisciplinary team to develop and implement individualized care plans, and ensure the delivery of high-quality and culturally sensitive services.
