Role Overview :
As a Primary Care Physician within our clients geriatrics program, you'll conduct comprehensive in-person assessments and collaborate with the interdisciplinary team to develop personalized care plans for each participant.
Reporting to the Medical Director, you'll oversee participant care in nursing homes and hospitals, including admissions, discharges, and coordination with medical specialists.
Additionally, you'll provide guidance and support to Nurse Practitioners and other team members.
Key Responsibilities :
- Conduct comprehensive assessments and provide primary care and continuous medical coverage to participants.
- Collaborate with community-based physicians and the team in discharge planning from acute or long-term settings.
- Actively participate in team meetings, communicating participant changes, collaborating on care planning decisions, and coordinating 24-hour care delivery.
- Maintain participant care in nursing homes, making regular visits as needed, and participating in rotating night and weekend calls.
Job Requirements :
- Completion of a residency program accredited by the Accreditation Council for Graduate Medical Education.
- Masters or Doctorate Degree.
- Board Certification in Internal Medicine or Family Practice (Geriatrics preferred).
- Active California Physician License without restrictions.
- State CDS Certification and DEA Registration.
- Valid State Drivers License with a clean DMV record and recent Vehicle Insurance.

Compensation :
- Base Salary : $240,000 to $275,000 annually.
- Full-time position with comprehensive benefits.
