Job Overview - Bilingual Case Manager :
Take on a vital role as a Bilingual Case Manager in New York, NY, providing crucial support to clients. Youll assess needs, coordinate services, visit clients, and manage case notes to ensure comprehensive care.
Compensation : $24 / hour
Location : New York, NY
Schedule : 9 : 00 AM to 5 : 00 PM, Monday to Friday
Responsibilities as the Bilingual Case Manager :
- Community Visits : Assess referred allegations and determine the need for APS services through client visits in the community.
- Case Documentation : Complete case notes, interactions, and progress notes to maintain accurate client records.
- Reassessment : Conduct monthly reassessments of client needs and revise case plans accordingly.
- Case Decisions : Render decisions based on APS eligibility criteria and address client inquiries.
- Service Coordination : Submit referrals for services to ensure client needs are met. Qualifications for the Bilingual Case Manager :
- Language Skills : Bilingual in Spanish, Russian, or Creole required.
- Skills : Computer proficiency, good writing skills, and strong time management.
- Experience : Ability to work with diverse adults who are mentally and / or physically impaired.
