Position Summary :
Community Health Worker will primarily be working in the Manchester & Nashua communies with racial and ethnic minority populaons.
CHW will work closely with the Granite State Organizing Project, the NH Alliance for Immigrants and Refugees and other local community partners.
CHW will conduct outreach to immigrant and minority populaons to encourage vaccine acceptance, including influenza and COVID-19 vaccinaons.
CHW may also work to encourage health literacy and leadership development. They may also facilitate referrals to address social determinant of health needs including economic supports such as the NH Emergency Rental Assistance Program and the Child Tax Credit.
Responsibilities and Authorities :
OUTREACH & CASE MANAGEMENT ACTIVITIES Works with residents to assess and address social determinants of health needs including economic supports and barriers to accessing supports Helps residents develop plans and goals and assists them in understanding instrucons and / or referrals Coaches residents in the importance of vaccinaons to prevent influenza and COVID 19 type viral infecons.
Documents acvies, service plans, and results in an effecve manner while strictly adhering to the policies and procedures in place, including through a shared database Conducts community-based or home visits, as necessary / requested, to reach residents Works collaboravely and effecvely within a community team that includes Amoskeag and Lamprey Health Centers, NH Alliance of Immigrants and Refugees, Manchester and Nashua Health Departments, GSOP and others
- EDUCATION ACTIVITIES Works with community team and supervisor to idenfy / modify culturally appropriate materials to use with residents to provide educaon about accessing services, and navigang local supports provided by social service agencies Assesses potenal resources to provide educaon to paents / residents such as : pamphlets, videos, posters, classes, one on one educaon Provides educaon about benefits of vaccinaons and economic support topics through financial educaon workshops that are culturally and linguiscally appropriate.
- COMMUNICATION Communicates informaon about vaccine safety and vaccinaon access to a wide range of community members, using techniques that enhance effecveness.
Uses social media, regular media verbal and wrien communicaon methods appropriate for each situaon. Provides presentaons to groups, as well as speaking one on one with paents / clients / community members.
OTHER Establishes posive, supporve relaonships with community members, including parcipaon in community / school-based events that may happen outside of typical business hours Helps people in ulizing resources, including scheduling appointments, and assisng with compleon of applicaons for programs for which they may be eligible Movates residents to be acve, engaged parcipants using techniques such as movaonal interviewing.
Effecvely works with people (staff, clients, doctors, agencies, etc.) from diverse backgrounds in reducing cultural and socio-economic barriers between communies and instuons Builds and maintains posive working relaonships with the community team Connuously expands knowledge and understanding of community resources, services, and programs provided
Qualifications :
- High School Diploma or GED
- Any combination of 2 years health / social services experience and / or educaon
- Verifiable good driving record and reliable transportaon with car insurance
- Background check
- Bilingual / bicultural, English and Spanish fluency
All CHW’s hired will be sent to the Southern NH AHEC Community Health Worker training program if they have not already aended.
Knowledge / Skills / Abilities :
In addion, individuals must possess these knowledge, skills and abilies or be able to explain and demonstrate that the individual can perform the primary funcons of the job with or without reasonable accommodaon using some other combinaon of skills and abilies and to posses the necessary physical requirements with or without the aid of mechanical devices to safely perform the primary funcons of the job.
- Physical requirements include ability to extend hand(s) and arm(s) in any direcon; pick, pinch, type or otherwise work primarily with fingers;

Primary funcons involve sing, as well as stooping, kneeling, crouching, and reaching; walking, parcularly for long distances, and standing for sustained periods of me.
- Visual acuity sufficient for work which deals largely with visual inspecon involving small anatomical or physiological details as well as the whole person, preparing and analyzing data and figures, accounng, computer terminal operaon, extensive reading, and visual inspecon involving small parts / defects.
- Excellent wrien, verbal communicaon and social skills.
- Ability to endure periods of heavy workload or excessive stress.
- Ability to wear equipment such as safety glasses, gowns, etc.
- Knowledge of database and spreadsheet soware systems. Computer skills preferred and basic financial skills, as well as analycal skills for trend and situaonal analysis desired.
- Ability to transport self to meengs, events and affiliated vendors.
- Not substanally exposed to adverse environmental condions.

Involves parcipaon in the formulaon and carrying out of policies, objecves and programs for major divisions or funcons. Considerable ingenuity and exceponal judgment required to deal with factors not easily evaluated, interpret results and make decisions carrying a great deal of responsibility.
Direct and coordinate the work of subordinate supervision in order to aain objecves.
Position Relationships :
- Directly supervised by the community organization and the Centro Latino Leadership Team
- Works closely with : A community team comprised of other CHWs from community organizations, Amoskeag Health, Lamprey Health Care, and the Manchester and Nashua Health Departments The staff of the NH Alliance for Immigrants and Refugees and the Granite State Organizing Project Other project partners regarding education, training, and referrals
Compensation and Benefits :
- A grant-funded posion at 32 40 hours per week through April 2022.
- Some scheduling flexibility is required.
- Salary range of $30,000 to $35,000 per annum.
- Non-exempt posion.
