Immediate need for a talented LTSS Service Care Manager (LVN). This is a 03+months contract opportunity with long-term potential and is located in Corpus Christi, TX (Remote).

Pay Range : $24 - $27 / hour. Employee benefits include, but are not limited to, health insurance (medical, dental, vision), 401(k) plan, and paid sick leave (depending on work location).
Key Responsibilities :
- LVN needed to assist with follow up calls to members, MD offices, DME agencies, Provider agencies, etc.
- LVN will need to work on any assigned tasks within expected turn-around time.
- LVN will need to track productivity and turn in spreadsheet each week.
- LVN may need to perform field visits in Corpus Christi and surrounding towns, if needed to assist.
- LVN will need to fill out required forms and document assessments within turn-around time.
- LVN must have good computer skills and be able to work independently in home office, being mindful of privacy to avoid any HIPAA breech.
- LVN must have stable and dependable Wi-Fi services.
- LVN must have good verbal and written communication skills.
- LVN will need to complete formal training, and then work within turn-around times.
- 8am - 5pm with 1hr lunch break; Meet with manager; Get started on training modules
Key Requirements and Technology Experience :
- Key skills; Managed care experience; Service Coordination; MD office work; Home Health work
- Good verbal and written communication
- Home health experience
- Comprehension of Microsoft software (excel; word; etc.)
- MD office experience
- Quick learner
- Education Requirement : LVN certification in BON
