Description
Essential Functions :
- Assists patients to maintain their sense of competence, identity and autonomy. Acts as an advocate for the patient / family.
- Demonstrates assessment / collaboration with hospital personnel and community resources to facilitate appropriate patient disposition while complying with organization and regulatory guidelines.
- Effectively utilizes community and regional resources to facilitate appropriate referrals.
- Participates in planning and continued process improvements.
- Assists in early identification of patients needing discharge planning intervention.
- Facilitates planning for patient / family needs to ensure a smooth transition for the patient across the continuum of care.
- Uses knowledge of community and regional resources, including benefits, eligibility standards, and qualifying criteria in developing a discharge plan, as documented in patient chart.
Uses age-specific standards to access services.
- Expedites decision-making process related to the discharge plan and / or counseling through collaboration.
- Keeps patient, family, and outside agencies informed of discharge planning activities and tentative / concrete discharge dates.
- Maintains documentation of progress and status of the discharge plan on patient chart on a daily basis.
- All other duties as assigned.

Required : Case Management
Case Management
Education - Completion of a nursing education program that is approved by a board of nursing or Bachelors in Social Works
Certifications - Registered Nurse (RN) - South Dakota Board of Nursing or Certified Social Work (CSW) - South Dakota Department of Social Services
Behavioral Health
Education - Bachelors degree in Human Services
Preferred :
Experience - 3+ years of Discharge Experience
Physical Requirements :
Light work - Exerting up to 20 pounds of force occasionally, and / or up to 10 pounds of force frequently, and / or a negligible amount of force constantly to move objects.
