Job Title : Occupational Therapist Job Specialty : Rehabilitation Therapy Job Duration : 13 weeks Shift : Monday-Friday, Days Guaranteed Hours : 32 per week Experience : New Graduates Are Welcome;
SNF Experience Preferred License : State License Required Certifications : Covid test 72 hours prior to start date with results issued 48 hours prior to start date Must-Have : - Completed Prescreen Form- Attached Resume- License Verification Job Description : Perform State License Verification- Provide Resume and Three References- Complete Background Checks : - County Criminal (last 7 years for all counties lived and worked) - Federal Criminal (last 7 years or more) - Multi-State Criminal Search (last 7 years or more) - Nationwide Sex Offender Search (last 7 years or more)- Conduct HHS / OIG Search- Conduct GSA Search- Run Social Security Number Trace- Complete 1 Step PPD or Quantiferon Test, or Provide Proof of Positive PPD and Chest X-Ray- Perform SAM Search- Obtain Virginia State Police Background Check- Fill Mandated Reporter Form- Submit a Sworn Statement- Position has normal urgency and does not require bilingual skills.
