Provide in-dept nutritional assessment and follow up for patients at high nutritional risk. Develops care plan including discharge recommendations, which may include instructions on appropriate diet therapies to patients / family.
Actively participates as core team member in interdisciplinary coordinated patient care program and on committees for the development and revision of nutrition related policies and procedures.
May assess patients to determine level of care and implement care plan in patients of moderate or potential risk. Acts as a source of nutrition care expertise to MMC staff and associates.
EDUCATION / SKILLS :

EDUCATION / SKILLS :
Master’s degree required for individuals who established eligibility for the registration examination effective on or after January 1, 2024.
Baccalaureate degree in Nutrition or related field from an accredited college for those that established registry eligibility prior to January 1, 2024.
Registered Dietitian (R.D.) status with the Academy of Nutrition and Dietetics (AND) : Registration Eligible status acceptable conditional on the taking and passing of the next AND qualifying examination for Registered Dietitians (within 3 months).
Maintenance of continuing education requirements mandated by the AND.
