The Transition Planner Clinical Services Specialist (TP CSS) provides client outreach, assessment, care planning referrals, and care coordination for clients.

The TP CSS supports transition efforts between various levels of care and system partners. They support clients by connecting them to medical, dental, mental health, and other service needs with a focus on addressing the patient's social determinants of health (SDOH) and as a result, a sustained diversion from the criminal justice system.

The Transition Planner Clinical Services Specialist often requires coordination with treatment teams and service providers.

Indirect services may include case consultation, group facilitation, education, research and selected assigned administrative and patient care duties.

Our comprehensive, multidisciplinary model is tailored to meet our patients' complex health needs. This position requires a skill set rooted in being able to meet the client where they are at and support with identifying the SDOH needs of the patient.

Duties of the Transition Planner Clinical Services Specialist included but are not limited to the following which is a representative sample of the level of work appropriate for position :

Direct Client Services and Care Coordination

The Transition Planner Clinical Services Specialist receives clients on referral, urgent, scheduled, or follow-up basis. Provide complex support for clients transitioning from a corrections facility to the community.

Responsible for initial psycho-social assessment, coordination and referrals for follow-up. Provides direct client support, intervention, counseling and client education, including how to access community resources.

Formulates definitive transition plan and documentation.

Provide ongoing care coordination and evidenced-based processes to assist clients remain engaged in services and / or connect with other service providers based on SDOH needs.

Assist clients with gaining access to a variety of social service programs as needed, including Substance Use Disorder (SUD) treatment, housing, peer support, medical services.

Provide direct assessment and crisis intervention for threatening and / or suicidal clients.

Asses client's level of suicide risk, mental health and ability to make own decisions / plans, cognitive functioning, and other safety and risk factors as part of transition plan.

Documentation and Reporting

Transition Planner CSS is responsible for the timely and accurate documentation of client care through the use of electronic health record charting.

Chart notes will involve detailed transition service plans, connection to providers, services, timeline and follow up actions.

This position utilizes Corrections databases to document and communicate housing and other logistical information with Corrections staff.

Participate in individual and team supervision and training to support the job functions of the team and self.

Track client assignments, collect TSP data tracking elements, write / develop reports to supervisory staff as needed to ensure professional services are being maintained by staff.

Actively adhere to policies and procedures, maintain client confidentiality and secure sharing of health information.

Develop, Foster and Maintain Positive Partnerships

Maintain partnerships and relationships with contracted agencies, health care providers and other social service providers.

Build mutually beneficial relationships with providers and community partners. Collaborate with the Corrections Health Programs.

Work with other Health Department teams, community partners and external agencies in an effort to offer and provide culturally, linguistically responsive programming.

Lead team(s) of staff for projects, process improvement functions, prioritizing and coordinating work assignments, and monitor work and service delivery to meet goals

Consultation

Engage in consultations with TSP multi-disciplinary team members and other members of Corrections Health for the purpose of troubleshooting, problem solving and ensuring a comprehensive transition plan for the client.

Use this coordinated care, partnership process and team approach to advocate for the client and collaborate in creating and updating individualized plans for each client based on their needs, transition plan.

Provide consultation to community members who contact the criminal justice system for the purpose of seeking guidance and assistance for friends and family.

Participate in meetings and consultations that may include family, support persons and other stakeholders. Consultations may become emotionally charged, threatening or bring forward other issues that have not been resolved or addressed.

Such intense meetings will require this position to provide options, practice de-escalation techniques, and hold attendees accountable for their behavior and actions, while providing a trauma-informed environment.

Minimum Qualifications / Transferable Skills* :

Education / Certification :

Must meet the standards of Qualified Mental Health Associate - (QMHA)OR Qualified Mental Health Professional - (QMHP) with the Mental Health & Addiction Certification Board of Oregon (MHACBO) - more information can be found at.

Only ONE of these certifications is required.

Current CPR / BLS certification (Valid certification upon hire)

May require a valid drivers’ license.

OTHER Minimum Qualifications :

The position requires a background investigation which may include being fingerprinted. Employment is contingent on passing a background investigation.

The employee holding the position is designated as an Essential Employee under Multnomah County Personnel Rule 3-15-015, Essential Employees.

Experience working in a clinical setting

Experience as a case manager or transition planner

Experience in a corrections setting working with a diverse population who display mental health and behavioral issues.

Licenses / Certificates : Professional licensure or certification ( Licensed Clinical Social Worker, Licensed Professional Counselor, Licensed Marriage and Family Counselor, Licensed Clinical Psychologist, Certified Involuntary Commitment Investigator).

Certification in Alcohol and Drug Counseling is highly desirable. (CADC III preferred)
